Novel biological markers are needed to personalize the treatment of patients with head and neck squamous cell carcinoma (HNSCC). In this study, Gessain and colleagues identified multinucleated giant cells (MGC), a subtype of TREM2-expressing tumor-associated macrophages (TAM), to be associated with longer overall survival. They developed a deep-learning model to automate MGC quantification on routine H&E/HES digital microscopy slides, facilitating its integration into clinical practice. Using spatial transcriptomics and proteomics, they revealed that TREM2-expressing MGCs and mononuclear TAMs clustered in keratin-rich granulomas. These findings challenge current knowledge about TREM2 TAMs and propose MGC density as a promising prognostic tool in HNSCC.
